I am 36 and noticing a lot of sagging skin on my face and neck. I think the sagging could be due to my 100lb Weightloss because there also seems to be extra loose skin (can’t really see it in my picture as it’s on my neck). Wondering if a deep plane facelift could help? I would personally prefer it to a mini or SMAS because I want the results to last a long time even if that means some scars.

Many patients experiencing significant weight loss will require neck lift surgery to eliminate the loose skin and redundant soft tissue in the neck. In most cases, deep plane neck lifting would be performed. Often, a traditional SMAS lift for the cheeks and lower face may be appropriate, even in those patients. However, the manner in which the face and cheeks would be addressed would depend on the amount of mobility of the neck and jawline. I've done many cases like yours and traditionally have been able to achieve great results in the face with SMAS or extended SMAS procedure. In the neck, it is likely that deep plane techniques would be required.

I almost exclusively perform deep-plane face and neck lifts. One of the many benefits of a deep-plane facelift is the ligamentous release and subsequent ease of deep tissue repositioning. Also, a deep-plane facelift does not increase the risk of scars compared to a mini or SMAS procedure, and in fact should decrease the risk because it does not rely on skin tightening but rather deeper tissue repositioning and therefore there should not be any tension on the wound. However, a deep-place facelift may not be what you need. You may benefit from a SMAS-procedure facelift and deep-plane neck lift for example. A consultation is needed to give you the proper recommendations.
