Wide circular nostrils can be narrowed and refined either as part of a rhinoplasty procedure, or as a separate procedure. Most of the time with an ethnic rhinoplasty, I am trying to create a more refined tip. Depending on the patient's goals and the physical limitation of the patient's cartilage and skin, once the size of the tip is set, I look at the amount of narrowing of the nostrils required to create a natural balanced nasal tip.

The tip lacks projection and the nostrils flare. All of these can be addressed with a number of maneuvers. Specifically for the nostrils you asked about an alarplasty or can be used to reduce the flare. However it is extremely important to address all aspects of the nose. Thank you for your question. Your nose can be elongated with various cartilage graft techniques. Your nostril shape can be changed but it is difficult to create straight nostrils without leaving you with a somewhat unnatural appearance. Seek a consultation with a BCPS with experience in African American and ethnic rhinoplasty. Good luck

While there are some limitations on what
can be achieved in ethnic rhinoplasty, it is possible to refine the nose and
make it appear more European. To narrow wide round nostrils some tissue is normally
removed from the base of the nose (alar base resection). Oftentimes cartilage
grafts can also be inserted to give better definition to the nose. You should
seek out a surgeon who has as lot of experience with ethnic rhinoplasty.

The shape of the nostrils is in part dictated by the underlying nasal skeleton.  Reshaping the cartilages of the tip will change the shape of the nostrils.  Often, in an African-American nose, transposing cartilage to enhance the projection of the tip is necessary.  Often, this can be accomplished without external incision.  From your description, it may also be necessary to reshape the alae which does require an external incision.
