At rhe momenr I wish i had tge opportunity to examine your skin ti evaluate your skin texture and skin laxity. there is its hard to advise. I would advise surgery for you. You need forehead rejuvenation with a modest eyebrow lifting and upper eyelid job with endoscopic approach. I purposely prefer endoscopic approach as the scar is almost invisible and very very limited for this technique. You have some hollow formation under lower eyelid and increased eyelid cheek distance. I would also add nano and micro fat grafting as well. Endscopic approach with midface surgery will address that too. The use of endoscopic technique dramatically reduce the risks associated with traditional facelifts, while significantly reducing the bruising and downtime of your recovery, from months to weeks.

Answer: Two ways, one non-invasive and one invasive It would be difficult to tell you for sure, without seeing you, but I see aging in at least two regions. From the pictures (I cannot see if there are "neck bands" due to positioning of the pictures). There seems to be some excess skin in the jowl and your eyelid, cheek junction appears to have aged. I cannot tell from the picture if there are bags under the eyes. I also cannot tell from the positioning if your brow has fallen or your eyelids have excess skin. I might asssume that you are quite attractive and wish to maintain that look in a natural manner. I might also assume that living in Hawaii, you have had some sun exposure and probably exercise. Without seeing you and assuming that all of the above are true, there are several options that you have. The most non-invasive one would be skin tightening by a non-invasive system such as Ulthera and filler injections to the cheek and area around your eyes. Botox might be needed also. There would have to be re-treatment at intervals. A middle ground would be a FaceTite/Fractora skin tightening. The FaceTite is a device that is introduced under the skin through needle holes and, via bipolar radio frequency energy tightens the skin. There is a body of peer reviewed literature that shows that is can tighten skin 30-45% by contracting the "fibro-septal network" under the skin, so it is effective. The skin is then treated with Fractora (RF microneedling) that treats the skin and heats it. Care needs to be done to avoid changing the fat structure of your face. Filler around the eyes completes the treatment. The best treatment is a well done structural facelift that lifts your structure and skin to a more natural position. This is a specific treatment that lifts the skin and muscle together (you probably have fine skin and do not wish to show "joker" lines) to give a natural appearance that "cleans up" your look. You also might need to lift the mid-face and eyes or brow (I cannot see this area). This would re-gain your youthful look without people noticing and would give you years of enjoyment. In short, there are options for you that simply did not exist formerly. Consult with a facial plastic surgery specialist to explore these options and more.
