You may need a combination of a skin graft, palatal graft, and canthopexy to correct your lid lag.  In the meantime you should continue upward massage on lower lid to help diminish the condition.

The lower lid retraction after lower lid blepharoplasty is a very complex issue.A thorough examination of the lower lid to determine the cause of the retraction, anterior lamella, middle lamella or the posterior lamella. The treatment is determined accordingly. Because the more surgery is done the more complex the repair becomes.If it is anterior lamella, skin graft can be done. The graft will show.Middle and posterior lamella a palate graft, if the posterior lamella is deficient.Trans blepharoplasty SOOF lift and canthopexy or canthoplasty, probably canthopexy.Based on the pictures posted may need combination of some of the above techniques.

In most case of ectropion (lower eyelid retraction). lower eyelid shortening will help. This however needs to be assessed during personal consultation. If the above is not possible, then skin grafting willl be a good option.

The suggested revisions you have listed may help.  Of course, you will need an in-person consultation to evaluate you for this procedure. Preoperative visualization and surgical simulation with the Crisalix system may help with your decision. I suggest that you find a board-certified plastic surgeon with whom you are comfortable. Be sure to have all of your questions answered during a face-to-face meeting with your surgeon, and review before and after pictures of patients who have had similar procedures. Good luck!
