Your surgeon can accomplish this by trimming, suturing, and reshaping the cartilage in your lower nose. Cartilage grafts may also be used to help improve tip refinement. Based on your photos, you appear to have relatively thin nasal skin. We can see that you have deep creases above your nostril margins which may be due to your previous surgery. This could also be associated with tip and nostril asymmetry. Basically, we need to restore where cartilage was removed in your nose. Too much was taken out which has led to buckling and folding of the remaining structures. That is giving you these "operated on" shapes and shadows. This can readily be done in about 90 minutes.
