I am 43 and am unhappy with my facial sagging. The area around my mouth/marionettes is my greatest area of unhappiness. My head is swimming from all the different types of facial procedures I have been researching. I would greatly appreciate professional recommendations!
Answer: DEEP PLANE FACELIFT There are different types of facelifts based on which tissues we need to mobilize. The most powerful technique that exists, is a deep plane lift. With this procedure, we can reposition all the tissues to where they were when a patient was in her 20s or, 30s ideally. The Deep plane method, releases all the ligaments that anchor the soft tissues to the Bone allowing us to rejuvenate and improve the definition and position of the jawline area cheeks and neck. Improve the position of mouth lines and, obviously the nasal labial folds disappear. Within the wide range of procedures, a deep plane face lift, solves all of these concerns. When in a situation where patients show signs of the aging process, categorized as, grade 1,2,or, grade 3, the same goes for all types of facial Rejuvenation procedures. Whether patients are in their 30s 40s, 50s or, even 60s sometimes, a patient's age is not a factor, but rather when a patient presents with the typical signs of aging, the best and most effective technique is, the extended deep plane face lift.

Answer: Full facelift Hello from Istanbul! Given your concerns, I would advise considering a full facelift to achieve the most effective and long-lasting results for both your face and neck. This approach addresses the facial sagging comprehensively, restoring a youthful look. Incorporating a modest cheek augmentation via fat grafting could further enhance and refresh your facial aesthetic. Hope this helps, best of luck on your journey!

Answer: Facelift/Necklift w/ Facial Fat Grafting Hello and thank you for your question. From what I can tell based on the limited picture you have provided, there is visible jowling with loss of jaw definition on at least the right side, and likely the left. You also have deep nasolabial folds, downturned corners of your mouth, and overall soft tissue descent in your face, especially noticeable in the upper cheeks. You have excess neck skin but it is unclear if there is platysmal banding present in your neck. Based on all that, you would definitely benefit from facelift/necklift. This would be the best way to address the jowls, excess neck skin, and overall effects of facial aging present, and to restore a more youthful appearance to your face and neck. A facelift or "mini-facelift" (a.k.a. short scar facelift) helps with skin tightening (by surgical removal of excess skin), but also allows for adjustments to some of the deeper face structures- e.g. tightening of the SMAS layer. The nomenclature can be confusing, but a facelift addresses the lower 1/3 of the face and the neck. That is why for someone with significant jowling and excess neck skin (as in your case), these are best addressed with some type of facelift. At the same time as the facelift, facial fat grafting can also be done to help further correct any asymmetries and restore volume to your cheeks and any other focal areas of concern. With all that said, it is difficult to provide tentative surgical recommendations, since there is only so much information to be gained from the single photo you have provided. It would be more beneficial for you to undergo a formal consultation to thoroughly discuss your surgical goals, undergo a formal examination, evaluate all your options (surgical and nonsurgical), and decide on the best procedure(s) for your specific case. I hope this helps! Best wishes, Dr. Donald Groves Plastic Surgeon

Answer: Why a traditional facelift may be right for you Thank you for your question and photo. You would likely benefit from a traditional facelift to rejuvenate your appearance and address the concerns you describe. This procedure can diminish facial sagging by lifting fallen tissue and improving your skin’s elasticity. Excess fat can also be removed to angulate your jawline and give your face a youthful, well-defined shape. I recommend scheduling an in-person consultation with an experienced, board-certified facial plastic surgeon. They will be able to discuss your concerns with you to help you decide on the best procedure.

Answer: Deep plane facelift Thank you for your question and sharing your photograph. In general, for management of the marionette lines and reduce the volume along with the jawline the deep plane facelift procedure seems to have more consistency in delivering the results. Depending on the characteristics of the neck, a short scar intervention could be eminently suitable. The procedure transfers volume from the lower face towards the mid-face and highlights the cheek projection which is an added aesthetic effect.
