While it is difficult to give you specific approaches to revision specifics because you are pulling at your labia with your fingers, which distorts your anatomy, it appears that you have had separation of a wedge-type incision. From my experience revising many such problems, there are two alternatives to improving the "look" of your labia (or a combination of these methods.) Simpler would be to "soften" the edges produced when the upper and lower portions of your wedge repair searated, by carefully skimming off the darkened edges in linear-type revision lines. The other way would be to "re-wedge" which, while more difficult would serve to better "pull up" the sagging edges. Both of these methods are feasible. The very most important part for you or anyone else seeking revision of their labiaplasty that separated or didn't turn out well is to consult with the best surgeon for your revision. WHO is the "right" surgeon? The best surgeon to make you "whole" is: 1) a very experienced surgeon who has done at least 200-300 labiaplasties, including several revision labiaplasties, and 2) ONLY see a surgeon who is savvy in BOTH linear and wedge-type procedures, so he or she can plan the procedure that is best for YOUR anatomy and YOUR needs. RealSelf's web reference below can help give you some ideas. While you may find the right surgeon closeby in your country, you may need to travel elsewhere for your surgery. In Egypt, that would be Dr. Amr Sefaldin; in Poland that would be Drs. Rafal Kuzlik or Dawid Serafin. In the U.K. that would be Dr. Alex Bader. In the U.S. that would be Dr. Gary Alter or myself. (**Of course there are other good revision surgeons; I have just named a few that I know well...) Very best wishes, Michael P Goodman, MD, FACOG, IF, AAACS. Davis, CA, USA
