Common reasons for lower blepharoplasty include tear trough deformity or pseudoherniation of the orbital fat. An in-person exam would provide a more accurate diagnosis, but preliminarily based on your photos it does appear you have pseudoherniation of fat creating "eye bags.” They should also be more prominent when you look up. Transconjunctival lower blepharoplasty with fat repositioning will provide nice results and a better lid-cheek transition in this case. The eye area needs significant combinations to get improvement. Lasers like erbium, co2, halo, profractional/contour, or Fraxel can all be used to build collagen long term. Plasma pen is a newer less invasive option but will take many treatments and require proper skincare. Plasma Pen is focused electrical plasma energy to tighten skin, producing small columns of energy just above the skin, allowing the skin to tighten to help improve wrinkles. This area will have 3-5 days of small “dots” that will then scab and peel, then the skin will tighter and brighter after. Numerous treatments are needed over lasers where only one or two give a bigger result, but more downtime. You mentioned that tear-through filler didn’t provide satisfactory results, but this is generally a favorable treatment option for adding subtle volume and reducing the appearance of bags and lines around the eye. Keep in mind that while filler treatments are not surgery, they do require a great deal of skill and precision to achieve the best possible results. Hello Madylina. Thanks for your question. You are a perfect candidate for Restylane injections under the eyes (see link). Because of your age and the fact that you do not have a lot of excess skin under the eyes, we would avoid surgery at this time. If you did not get a good outcome from the under eye injections previously, perhaps it is because the practitioner doing the work was not experienced in this particular area. It is one of the most difficult areas of the face to inject well. We prefer only Restylane in this area as it has the proper consistency and water absorbing properties.

An examination of you would provide the most information to give you advice about your under eye area, but just based on your photos, you appear to have some fullness that could be due to fat under your eyes. If this is the case, this issue can be addressed with lower eyelid surgery. In many cases, fullness under the eyes can be due to hereditary causes and not due to aging.

Since the upper cheek region acts as a major support for the under-eye area, when there is volume deficit in that area not only do the cheeks suffer, but the under-eyes, do as well, with the formation of tear troughs and bags, etc. The use of an appropriate robust, natural HA filler injected properly by an experienced injector can not only nonsurgically enhance a more feminine oval face shape, but can significantly improve the under-eyes, as well.
