Custom silicone implants. Will 800 saline implants give a bigger look than 800 silicone?

Although 800ccc  should look the same generally saline looks bigger because it is less pliable and stiffer. Also you can overfill saline implants - they will loose warranty and possibly will look/feel very fake. I would not advice you to get  larger silicone implants - first of all they may not have the same quality  as Mentor/Allergan and moreover large weight of the implants may pose more risks for the future - thinning of the breast tissue, bottoming down etc

I would echo the response of other doctors on this website.  Regardless of silicone versus saline, 800 mL looks like 800 mL.  I cannot comment on custom silicone implants as they are not available in the United States.  In general, overfilled saline implants feel quite firm.  The more the implants are overfilled the more firm they feel.  Going outside of the manufacturer's recommended fill range does void the warranty on saline implants.  I would caution you to be careful about going out of the country or going to see someone in the country who will do something that is not generally accepted or legal here.  I hope that you can find what are looking for.  Extra-large breast augmentations can be tricky.  Extra large volume implants create some tremendous forces on the breast tissue over time.

Currently, your only way to get above the 800cc range is to use an 800cc moderate plus saline implant and overfill it to the volume you are looking for.  The manufacturer range on the 800cc is up to 1000cc.  However, everyone knows that it can be filled to 1500cc and still feel somewhat soft and completely maintain its shell strength.  Unlike a custom silicone implant ( which is not allowed in the US), the saline implant overfilled gives you flexibility during surgery to add more if the tissues can handle it.  I have patients with 2700cc and love them.  As a test I have taken an 800cc Shell and filled it to 3300cc.  It looked like a basketball, but we dropped it on the ground ten times and nothing happened to it.  The best thing to do is have a consultation and see what will work best for you.  I hope this helps.

800 cc is just that 800 cc, silicone, saline, or whatever material is used to fill the implant. Silicone feels more natural than saline, even overfilled saline.  Remember, if you overfill a 800 cc saline implant to over the manufacturer's recomendation, then you will lose your lifetime warranty on that implant.Good Luck!

In the US, the silicone implants go up to 800 cc's, and the saline can be overfilled to 1000 cc's with the company's recommendation, but some physicians choose to overfill beyond that size. The overfilled implants will feel harder. Do remember, that your tissue must accommodate this large size and your frame will have to carry the weight. Please do not go beyond what your frame and tissue will support, or long term it will not look good, will sag, and then may issues with tissue that is too thinned.

In the U.S., 800cc (of any profile) is the largest volume implant legally available. Most of us who regularly do oversized implants will use an overfilled 800cc moderate-plus saline implant (usually need the dimensional width). We sometimes go as large as 2000-2500cc. This clearly results in a much stiffer, firmer implant compared to a silicone gel implant.800cc saline and gel of similar profiles will have little, if any, meaningful difference in appearance. Custom implants are not legally available in the U.S.

There afre several ways to get the fake look-either with overfilled saline implants, custom XL silicone implants or XL tissue custom expanders.It depends really on what cc size to want to get.
You can think of a saline implant as a balloon. The manufacturer recommends an overfill of about 20cc but they have been shown to be able to be filled much more than that. They do start loosing their shape and can significantly distort the appearance of your breasts. 
 I do overfill espcially since the FDA has restricted saline and silicone implants to a maximum saline shell size of 800cc and maximum silicone size of 800cc. I do not know why that number was picked arbitrarily. 
So, I do do breast reconstructions greater than that with XL tissue expanders which do pride a greater diameter and projection and upper pole fullness and allow the size to be adjusted either up or down. Also, the shell of the saline implants are very thin and can dreflate more easily.
