I dread the idea of getting an implant let alone scarring on top of it. So many things can go wrong but I’ve also always hated the shape of my breast and have been looking for almost 8 yrs. Finding the right doctor who is knowledgeable, skilled and attentive is hard. I know each patient is different therefore their results are. So here are my breast and an idea of desired breast. I would prefer a lift through my nipples if possible because I want an areola reduction and for them to be more even.
Answer: Breast lift with implants Hello and thank you for your question. The best advice you can receive is from an in-person consultation. With that being said, based on your photographs, the best shape and contour could be achieved with an implant and a vertical or wise pattern breast lift. I rarely recommend a Bennelli or circumareaolar (donut) lift. Over time, the areola tends to enlarge with a thickened scar. The other even worse problem with a circumareaolar lift is that it flattens out the shape of the anterior breast over time. A vertical or wise pattern lift with an implant can result in a very nice shape if good technique is used. The size of the implant is based on your desired breast size/shape, your chest wall measurements, and soft tissue quality. This decision should be based on a detailed discussion with equal input from both you and your surgeon. Make sure you specifically look at before and after pictures of real patients who have had this surgery performed by your surgeon and evaluate their results. The most important aspect is to find a surgeon you are comfortable with. I recommend that you seek consultation with a qualified board-certified plastic surgeon who can evaluate you in person. So much so, I can't imagine even the most brazen of providers who perform breast surgery would offer that surgery to you. All the usual problems would likely occur to you: 1. thickened irregular scaring around the areola. 2. Enlarged and irregularly shaped areola. 3. Pleating of breast skin around the areola. 4. Persistently low areola. 5. Flattening of the breast mound. 6. Persistent sagging of the breast mound. A periareolar lift would be a disaster, a travesty. Your breasts are heavy, asymmetric in size and shape, and very sagging; your goal breasts could only be obtained with a formal lift, asymmetric reduction, and implants. Go visit a few ABPS certified/ASAPS member surgeons who specialize in breast surgery. Best of luck!

Answer: Can I get a nipple lift with implants to avoid scarring? This is a question I get asked often in my practice. Based on the long distance from your sternal notch to your areola and the degree of breast ptosis (droop) that you have, a circumareolar mastopexy is not recommended. This type of lift tends to result in less favorable scars and tends to flatten the front of the breasts. You would have a more aesthetically pleasing and effective lift with a full mastopexy. Although many women have reservations about having a vertical scar on the breast, this scar tends to heal very well in the vast majority of patients. I would consult with a few board certified plastic surgeons in person to discuss your options in more detail. Best of luck!

Answer: Nipple lift to avoid scarring? Thanks for your question! Based on the amount of ptosis (droop) that you have, I think you would benefit from a true mastopexy or breast lift. After seeing you in person, I could determine if we could get away with doing a lollipop or vertical mastopexy or we would need to do a full wise pattern (anchor incision). You may find a surgeon willing to give you what you are asking for, but I don't think you would be happy long-term. Good luck!
