Yes. Your breast implants have bottomed out. As far as doing a revision before or after kids, I’m not sure it matters. You don’t have much natural breast tissue, so your appearance shouldn’t change much after pregnancy. I have had two kids and have still not changed out my implants.

It is difficult to determine what exactly is going on without a proper side by side comparison of before and after photos. This information would also need to include the size of the implant and pocket position.

Waiting until after you have children is not necessary. Your implants are bottoming out and you may benefit from smaller implants, additional pocket dissection on the upper portion as well as inferior plication.

Aloha and thanks for the photos. It appears the septal structures at the inframammary border of the 5th ribs were released, and this is causing the descent of implant position. You don’t need to wait until after having kids. As long as the tissues are fully stable, usually 6 months or more from the last procedure, revision with capsule repair can be done. Otherwise, a well structured underwire bra is needed to support the implants higher against gravity.

This is a great question. Based on the pictures shown, it does appear that the breast implants have bottomed out a little bit. You can tell by noticing there is more volume underneath the nipple/areola, and, the distance from the nipple to the breast crease appears longer than what is appropriate. The upper part of the breast/chest is also somewhat deficient because of the implant settling a little too low.

In terms of when to have them removed/replaced is up to you. If you are considering increasing the size, it is important to keep in mind that if you are planning to have children within a year, your breast size will increase due to the hormones of pregnancy and if you breastfeed it will continue to increase in size. That increase in size can become uncomfortable in patients who recently have large/larger implants placed. If you are thinking of downsizing or even staying the same, it may not be as much of an issue, but keep in mind the swelling from the surgery may play a role also in terms of size. Hope this helps!

The decision to pursue revision surgery before or after children is a purely personal decision. A revisionary surgery for this problem would be relatively straightforward with implant removal, reinforcement of the IMF with sutures and possible an absorbable mesh and superior capsule release. You may still have changes to your breasts should you have children in the future.

You have bottomed out some. If you want kids, go ahead and head them. You may need more adjusting once you are finished having all the children you want.
