Hanging columella and possible deviated septum ? How much ? Recovery? Will I drastically change appearance? (photo)

Dear Jess:  Rhinoplasty (nose reshaping surgery) to address a wide nose and hanging columella is a common request. Most likely, an “open rhinoplasty” (where the soft tissues are gently elevated to allow direct access to the tip cartilages) will be required. These tip cartilages are then sculpted and sutured to refine the nasal tip. Osteotomies are required to reposition a wide nose closer to the midline.Septoplasty may be performed concurrently to address breathing issues.If you like, please follow the link below.  A deviated septum can only be diagnosed with an internal examination of the nose. Once medical necessity has been documented, and you have failed medical therapy, consideration for a septoplasty is in order. There are many other issues inside the nose can also cause nasal obstruction. A septoplasty will not change the shape of the nose. A hanging columella is reduced  by removal of skin and cartilage from both sides septum on the inside of the nose. For many examples, please see the link and the video below

small changes to the nose can change the way a person looks. The cost of
a rhinoplasty varies from patient to patient depending on the type and
complexity of the procedure. Rhinoplasty consultations should be
done in person. A thorough exam is required as well as a lengthy conversation
regarding your goals as well as your convalescence. I recommend that you make
an appointment with a board certified plastic surgeon to discuss your surgical
options.

Your problems are all due to septal overgrowth causing the deviation, the tip problems, the upper lip shortening and stretching and the hump on the lower bridge.  An approach inside the nose without grafts to reduce the overall size of the septum will have the most natural and aesthetic results, as well as functional improvements.

Your hanging columella can be elevated, the tip refined, the septum and the nose straightened with a septorhinoplasty procedure. I strongly recommend that you consider reducing the  hump, the only part of your nose that you're not improving. It will add little to the surgery and you will achieve a natural, non-surgical result as long as you work will an experienced rhinoplasty specialist.

I think you are making a mistake to tie the surgeon's hands from working on your bridge. Your nose is too large because of the high, humped dorsum. That, and the nonprojecting tip that you are just narrowing, your nose will continue to detract from your good looks.
