Tight lower pole, a high fold, limited inferior muscle release or the way the capsule has formed can all hold the implant up. Strong pectoral tone can contribute yet it is rarely the sole reason.
Why the lift did not solve itA lift repositions the nipple and skin envelope. It does not move the implant. When the pocket still holds the implant high the breast tissue can settle over it and the shape looks similar again. This is the classic waterfall pattern.
What a successful revision focuses on• Pocket reshaping. Lower the inframammary fold in a controlled way if it sits high. Use precise capsulotomies or limited capsulectomy so the implant can occupy the lower pole• Muscle management. Adjust the dual plane and release only where tightness distorts shape. If animation deformity or persistent high position is expected consider conversion to subfascial or prepectoral when tissue thickness allows• Internal support. Capsulorrhaphy, internal sutures or mesh can stabilize the new fold and prevent the implant from riding high again• Sizer use intraoperatively to confirm symmetry and cleavage before closure

A larger or extra high profile implant can increase upper pole fullness and projection yet it may also push laterally if the medial pocket is not released and controlled. Over release medially risks symmastia. Your base width is 14,6 cm. An implant must match footprint and the plan, not just chase the number. Cleavage is achieved by safe medial control, not CCs alone.
What to clarify with your surgeon before revision• Which finding is the primary driver of the high position on the left and right• Exact plan for the fold level on each side and how it will be stabilized• Capsular work to be performed. Capsulotomy versus partial capsulectomy• Plane choice after revision. Stay dual plane or convert to subfascial or prepectoral based on pinch thickness• How medial pocket will be shaped while avoiding symmastia• Whether sizers will be used and how symmetry will be confirmed on the table
Expectations and safetyA well planned pocket revision often corrects a high implant. Results depend on disciplined fold control and soft tissue support. Size escalation alone rarely fixes cleavage and can add weight and stretch. Standardized photos and your operative notes allow a more precise roadmap. breast revision surgery is the most difficult plastic surgery procedure performed. Botched breast surgery alters normal tissue planes and laxity, the ability of breasts to heal properly, and leaves scar tissue that dramatically affects the level of difficulty of the corrective surgery.
Many patients come in to correct poor outcomes from their initial surgeries. The most common problems are due to implant malposition, bottoming out of the implant, poor scarring, and incorrect implant size.
While the type of revision required will depend on the result of the previous surgery and the desired result of the patient, some revisions involve extensive pocket repair that may or may not need specialized external bras.
If you are considering breast surgery revision, you should speak with a board certified plastic surgeon and have yourself properly assessed. Only after a thorough examination you will get more information and recommendations. Also, you have to make sure that your surgeon understands your breast augmentation goals.
