I have done my boob job in miami 9 years ago and in my recent ultra sound i found out my left breast implant is destroyed from inside: i have consulted 3 drs and two have advised me to replace the implant with new with mayve moving one capsule beteeen the another dr recommended to completly remove the capsule and didnt recommend to chage it immeidlty as he said its risky. This is a high-priority situation necessitating immediate surgical intervention. I recommend urgent explantation (removal of the implant and its surrounding capsule).
1. Pathology Protocol: If the implant brand is identified as Allergan, it is critically necessary to send the following specimens for specialized pathological evaluation to rule out BIA-ALCL:
• Capsule: For detailed histopathological examination.
• Periprosthetic Fluid (Seroma): For cytological examination and CD30 immunohistochemistry.
2. Reconstruction: Following explantation, if the residual breast volume is equivalent to a D-cup, the reconstructive plan will involve a mastopexy (breast lift) combined with a replacement prosthesis to achieve an optimal aesthetic and elevated contour.

When a silicone implant has ruptured it is generally advisable to remove the implant and capsule.  If the silicone is outside of the implant but still within the capsule you can consider removing them en bloc.  This refers to removing the implant and capsule as one unit.  This minimizes the risk of the silicone escaping to surrounding tissue.  I've included a video I hope you find helpful.
