Although you are very thin and don't have a lot of extra skin...not enough for a traditional tummy tuck...it is likely some extra skin that has caused your belly button's appearance.Perhaps consider a tummy tuck where your belly button is floated...meaning that it is detached from the deeper muscles but not scar is place on the skin around it. A more modes amount of skin is removed and the belly button position is lowered a couple centimetres. Improves appearance of belly button, flattens tummy, provides opportunity to repair any muscle separation (takes away bulging), has no visible scars when wear a bikini or underwear.
Dear Sadbelly, I know your problem because my BB looks just like yours. You see, I was much heavier when I was a child so my skin stretched with my belly. When I grew up I leaned up but the stretched skin made my BB stalk very long so when the skin around it sags it stretches the long stalk making it look like a frown, I've seen lots of those. Even after loosing weight and having had liposuction, the lenght of the stalk won't shrink. you can tell that the skin in the center of your tummy around your BB is the loosest. Unfortunately I don't know of any other way to correct this except for a tummy tuck which allows to reduce the lenght of the BB stalk and therefore make you a nice round and petit BB. I understand you not wanting to have a TT because you're already thin and you don't really have a lot damaged (redundant, loose, stretch marks) skin on your tummy

Thank you for asking about your navel piercing.A plastic surgeon should be able to remove the stretched skin from the piercing and suture the area closed. This leaves a small scar that becomes unnoticeable in time. Always see a Board Certified Plastic Surgeon. Best wishes - Elizabeth Morgan MD PHD
