If you did not have the pocket revised at the time of the revision there is not a good chance that the cleavage will improve.  In fact now that there is a larger implant in a fixed pocket space, it will look worse than prior to revision because the pocket wasn't altered.

Good evening,I agree although the lift was done well that your gap is really wide and draws your eyes. I would recommend modification of your pockets so that they are open more medically, and then switch implants to ones with a wider base diameter, perhaps in the 600cc range.Hope this helps!

I have had good results improving this situation by doing fat grafting to the inner sides of the breasts.The advantage is that it is a simple procedure and can be safely done with implants in place, without having to move the implants. It does not need much fat to get a significant improvement.

You have some bottoming out of the implants with continued stretch or strain on the lower breast pole. This is due to excess weight of the implant and thin/inelastic skin. I see this more in thin women with larger implants. You will likely require additional support with either Strattice or Seri. Adding a wise pattern or horizontal incision may be necessary as well. The internal pocket or capsule can be tightened with a capsulorraphy but this can also stretch with time. Overall you have a much better result and a bra can cover up this lateral drift. Your breasts are getting farther and farther apart over time because the bottom of your breast is drifting lower and slightly to the outside.  This unfortunately is one of the known complications associated with the lollipop breast lift.  If you look at your breasts in the middle picture above, there is much more fullness above the nipple.  As the implants drop down into the pocket farther and farther, the cleavage separates more and more.  Technically, this is referred to as capsularptosis.  This is correctable by one of the many techniques to close off the bottom of the breast implant capsule (pocket), and very often these days, a hammock is created by Seri to decrease the chance of recurrent capsularptosis.A second technique to absolutely insure no more implant fallout in the future would be to add a horizontal component to the lollipop lift making it an anchor or standard mastopexy.  At this point, it is appropriate to seek an in office visit with your plastic surgeon to correct this ongoing ptotic problem.

You don't mention what the time frame is since your surgery.  But if you are over three months postoperative, you will likely need a revision to improve the appearance.  The implants will likely need to be moved inward and up based on your photos.  However, an in person exam would be needed to confirm this.  In order to move the implants inward, the lateral pocket will need to be closed down and reinforced with a material such as Strattice, Alloderm or Seri.  To raise the implant up, the vertical mastopexy may need to be converted to a Wise pattern (or anchor) type scar to remove excess skin from the inferior pole of the breasts.  The inferior portion of the pocket may need to be reinforced as well.  

I am sorry to hear about your concerns after breast surgery. Based on your photographs and description, I think that, at some point, you will likely benefit from additional surgery. This revisionary breast surgery may involve suture "repair" of breast implant capsules (capsulorraphy) and/or additional breast lifting surgery. This type of revisionary breast surgery will serve to move your breast implants higher on your chest wall and better center the nipple/areola complexes higher on each breast mound. Make sure that your plastic surgeon has a significant/demonstrable experience helping patients with these types of revisionary breast operations. Best wishes.
