I have had lower face lift, neck lipo, upper & lower bleph & co2 laser a year ago. I am left with malar bags, festoons,. Makes me look very old & tired. It was told to me that the laser treatment wd get rid of these. Anyone have a solution for this? Thanks
Answer: Malar Bags/Festoons You do, indeed, have malar bags or festoons. As in your case, these can, unfortunately, detract from one's appearance and, as in your case, they are not improved by facelift or blepharoplasty. There are very few doctors who have experience addressing this problem and even fewer who are good at it. Contrary to advice that you have received from other doctors on this forum, avoid fillers at all costs and personally, I would avoid any doctor who advocates for direct incision over the bags. There are better options! Only see an experienced oculoplastic surgeon who can show you before and after photos (the more the better) of other people he or she has treated. In person consultation (few) with a board certified plastic surgeon with good reputation in eyelid surgery/facial rejuvenation is recommended. Good luck.

Any non-surgical treatment is also highly unlikely to give you a great result and fillers can sometimes make these look worse since they can block some lymphatics and cause additional swelling in the area. I typically reserve direct excision for festoons that are severe and almost are a flap. For festoons like yours I prefer the extended lower blepharoplasty with midface lift. Answer: Post lower bleh malar festoons the best solution is to use a laser like Resurfx and get fillers such as Sculptra in the cheeks. Regards

Answer: Malar bags Malar bags are difficult to treat. Some patients see improvement with lasers, but I don't think you'll notice a significant difference based on your photos. I recommend visiting with a board certified plastic surgeon in your area to discuss your concerns and explore options available to help address this area.
