Dear Doctors , I had mastopexy with implant under the gland in turkey 2 years ago . Recently i contacted my doctor with no reply from him . I wanted the implants to be under the muscle however he said because my breast are big and we have a reduction he will put them under the gland . Obviously i did not know . Now i have loose breast you can tell i have implants and in left breast i have pigmentation . I am thinking to take them out in uk and put the implant under muscle ? What are ur suggestions?
Answer: Revision I encourage you to visit a board certified plastic surgeon near you for a consultation. It appears a revision to the lift or at minimum a scar revision would be helpful. The implants can be exchanged and placed under the muscle at that time.

Answer: Breasts I always recommend placing the implants behind the muscle in a lift or reduction to help give the implants better support, better coverage, and less sagging potential. Your already stretched out tissue from large breasts does not support the weight of implants well and will just sag. I recommend going down in size as well for the same reason. Find a doctor in your area for better follow up care.

Answer: Mastopexy Revision It appears that the implant position is not your primary problem. Certainly, if you would rather have subpectoral implants, that can be done. Your main issue seems to be your lax and ptotic soft tissues. A revision of your implants can certainly be performed (either by contouring your current pocket or by creating a new dual plane pocket, likely with reinforcement material) but just by redoing the mastopexy you should have a far superior result. Given the way you scar, I would take certain precautions regarding how sutures are placed as well as postoperative incisional care.

Answer: Explant with Bellesoma Method I recommend explantation and lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery.
