Have my implants bottomed out again? (PHOTOS)

A year ago I had a breast revision to address bottoming out. The surgeon removed the previous implants that were 200cc and 250cc and replaced them with 320 and 350cc. However i feel like they have bottomed out again. My areola sits so high and any sort of bra/ low cut top i wear they end up showing as they're so high. What are my options?

From the photos, the areolas do appear high on the breast mound with more fullness and stretch in the lower pole, which can be seen with recurrent bottoming out or loss of support at the inframammary fold. The key exam findings are whether the implants sit below the intended fold and whether the nipple-to-fold distance has lengthened compared with your early postoperative photos.

If this is recurrent bottoming out, options usually involve revision of the implant pocket rather than simply another lift. This may include raising and reinforcing the fold, capsulorrhaphy/internal bra support, mesh or ADM in selected cases, and often using a smaller or lighter implant if the tissues have stretched. An in-person consultation with a plastic surgeon experienced in revision breast surgery is important, because the best plan depends on your tissue quality, scar position, implant size, and whether the fold can be reliably rebuilt.
