I'm 61yo female 5'4 and 135lbs. i know i need a lift and implants to make both breast the same size. . most lift pictures i see it just looks like the nipple is moved up and the breast is folded and still saggy. the aesthetic I'm looking for is perky where you can see the inframammary crease and i want upper pole fullness. i want to be a perky c is this even possible at my starting point

You can achieve this at 61 years old, with the proper plan in place. Most lifts you saw appeared saggy and folded because the surgeons treated the nipple and forgot the lower pole skin and upper hollow. You skip an implant to achieve that clear inframammary crease or the upper fullness, instead, you end up with a nipple only Mastopexy, which in your tissue type returns to saggy form within one year. I will suggest an anchor pattern lift with a moderate plus or high-profile silicone implant, the diameter sized appropriately for your base width and carefully measured to match between breasts for balance. With this, the implant defines the IMF and your upper pole. The lift will take out excess lax skin, lifting the breast tissue up over the implant. At your tissue type, I usually incorporate a GalaFlex Lite mesh to sling below the gland and support the outcome. Getting a perky C with a fair amount of upper fullness looks possible.
