Age 33 - I have a double chin I'm unhappy of. While I believe I may have a slightly recessed chin, jaw surgery is excessive, and a chin/implant or genioplasty would worsen my chin crease. What I dislike most is the little sagging pocket of fat (or skin?) directly under my chin, in addition to not having a jawline - unless I tilt my head up. Would chin/jaw/neck lipo benefit me? Or a neck lift or platysmaplasty? I'm aware individual anatomy affects outcomes. X-ray is attached.
Answer: Is Chin Liposuction the Right Choice for Me? Chin and neck liposuction can be a great option if your main concern is excess fat under the chin and along the jawline. However, its effectiveness depends on factors like skin elasticity, underlying bone structure, and the degree of sagging. Here’s how to determine if chin lipo is right for you: ✔ Good Skin Elasticity – If your skin can retract well after fat removal, liposuction alone may be effective. If skin laxity is present, additional procedures may be needed. ✔ Isolated Fat Deposits – If the "double chin" is mainly due to excess fat rather than loose skin or a recessed chin, liposuction can provide noticeable improvement. ✔ Mild to Moderate Sagging – If you have significant skin laxity, a neck lift or platysmaplasty may be necessary to tighten the area after fat removal. Since you mentioned concerns about a recessed chin but do not want an implant or genioplasty, subtle contouring with liposuction alone may help. However, for a more defined jawline, radiofrequency skin tightening (e.g., FaceTite, Morpheus8) or a neck lift might provide better long-term results if sagging is present. A consultation with a plastic surgeon can provide a customized recommendation based on your anatomy and goals.

Answer: Digastric muscles volume reduction - anterior neck lift Thank you for your question and for sharing your photographs. The concern you have described is most likely related to the volume of the digastric muscles beneath the chin. These can be reduced, and the procedure that typically addresses this—along with any additional deep fatty tissue in the submental region—is an anterior neck lift. It is also worth noting that weight training can sometimes contribute to an increase in muscle volume under the chin due to the strain placed on the floor of the mouth and tongue during intense effort. An anterior neck lift is generally effective in addressing these concerns and achieving a more refined contour.

Answer: Neck liposuction may be considered, however, you may not have a lot of fat in your neck. Neck liposuction is a minimally invasive procedure that can improve the appearance of somebody's jawline, and also improve the appearance of a double chin. It's a bit unpredictable when there is little fat in the neck. We have performed procedures on patients who look similar to you, and have had mixed results. Please consider meeting with several reputable facial plastic surgeons so you could see what to expect. An examination would be helpful for determining your candidacy. Thank you for this question, and I hope it's helpful. Sincerely, Dr Joseph

Answer: Neck fat and loose skin Chin Liposuction and Thermitight will be the best option Chin liposuction helps reduce the bulge under the chin and can be done for the whole neck. Once the fat is removed the skin can become loose and saggy in some cases. It can be avoided by combining it with Thermitight which is a radiofrequency heat delivered under the skin to tighten it. Both chin liposuction and Thermitight can be done at the same time. The recovery is fast and it can be done under local anesthesia. Click on the video link below Regards Dr. J

Answer: A rock and a hard place. Everything you included in your statement is correct and accurate. The problem is fundamentally based on facial skeletal bone structure, not soft tissues. It’s possible to have a projecting chin on a small mandible. Individuals whose lacking in projection are prone to developing a premature double chin and loss of jawline definition. Liposuction can give good improvements. There are basically three variables that determine someone’s Liposuction potential. The first is age which intern determines the degree of skin elasticity. The second is the amount of fat available to work with and the third and most important is provider selection. You look pretty young so I think your skin elasticity is probably excellent. You’re not chubby so there isn’t that much subcutaneous fat to work with. Provider selection is by far the most important variable and finding the right plastic surgeon is not easy or straightforward. Before even contemplating surgery, you should realize that applying soft tissue solutions to a problem that is based on bone structure is at best going to give you a partial improvement. Looking at before and after pictures always have your own pictures available to use that reference. Look for examples of previous patients who had very similar facial characteristics. That should mean they are a young man with the same type of bone structure and the same amount of subcutaneous as fat. Personally, I don’t think a platysma plasty or any other treatment is going to do too much for you. I wouldn’t get too heavy-handed with soft tissue surgery that is only going to give a mild improvement. Later in life and neck lift may be an option, but it’s not at your current age. You should at least talk to oral surgeons about form jaw surgery, though you may not want to go that route and I can understand why. Sometimes the best option is to just stop focusing on the problem and go on with life. There is no simple procedure that’s going to give you a great improvement. if a small change is justified for plastic Surgeons who have extensive experience with chin and neck liposuction. Finding the right provider is not at all easy or straightforward. In my opinion, it requires having multiple in person consultations. During each consultation, ask each provider to open up their portfolio and show you their entire collection of before, and after pictures of previous patients who had very similar characteristics to your own. Always bring a proper set of facial pictures of yourself to use as reference preferably imprinted format. Don’t rely on memory of what your face looks like when you reviewing before, and after pictures. Take carefully notes during the consultation, especially regarding the quality and quantity of before and after pictures. Do this for whatever procedure the surgeon recommends. If they give you options, then ask them to show you before and after pictures of each of the different procedures, they are suggesting. highly experienced surgeons should have no lack of before and after pictures. If they don’t have a quality collection of before and after pictures and then that’s your first red flag to keep looking. I think ideally they should have at least 10 to 20 sets before and after pictures. Highly experienced surgeons should have access to hundred of before and after pictures to choose from. Make sure the neck angle is the same and before and after pictures. People can lead to temptation surgeons are no different. It’s pretty easy to manipulate the results by simply asking the patient to tilt their head back adjust a little bit. There’s no correct number of consultations needed to find the right provider. The more consultations you have the better of an understanding you’ll have and the more likely you are to find the better provider for your needs. When down slow down and scheduled more consultations. If you can really find the right provider, the Liposuction may be worthwhile. Average plastic surgeons give average result and average Liposuction in your case isn’t going to be all that great. This is a finesse procedure and just going in and sucking out fat is not going to give you the results you’re looking for. Sometimes this simply isn’t a good solution and you should always keep that in mind when Contemplating a permanent irreversible surgical procedure. Best, Mats Hagstrom MD
