Hello @Jjo3, thank you for your question. It really depends how the explant is done, surgical procedures vary from surgeon to surgeon. The explantations I perform are definitive, that is, the prosthesis is removed as well as the capsules and the breasts is reconstructed with the existing tissue the patient has in order to achieve the best aesthetic result. The breast reconstruction ensures that the breast doesn't look deflated, since removing the prosthesis will result in repositioning of the mammary tissues. The most appropriate course of action is to consult one or several board certified plastic surgeons. This will allow you to get more information and options tailored specifically to your needs. When you remove the implants you will lose the fullness that they provide. The skin will retract to some degree. It will be influenced by the elasticity of the skin and the length of time you have had the implants in place. Your breasts will be smaller and a more natural shape with much less upper breast fullness. They will not look like they did prior to surgery since the tissue has been stretched and thinned with implants.

If the capsules were not thickened enough to require removal, then there should be no problem leaving them in when placing new implants. They will not interfere with future examinations. However, if you had textured implants, be sure to discuss whether to have a capsulectomy.
I prefer to have my patients explant only and I will close with a psuedo lift at the same time. This way the contour is natural looking. After a years time is when you see how the breast tissue fills out and fluffs. My patients are very happy with their outcome after the fact.
Ultrasound will tell if the implant is ruptured, which will be useful information because it will be covered by warranty if so. You will probably have a capsulectomy if you have capsular contracture, and you surgeon will determine if there is anything of concern.
