A higher profile implant can add more forward projection for a given base width, but it does not reliably create cleavage without a bra and it does not truly tighten loose skin. Cleavage depends on your natural breast footprint, the distance between the breasts, chest wall shape, implant width, pocket placement, and how much soft-tissue support you have. In the photos, there appears to be skin and breast tissue laxity with a lower nipple position, so a profile change alone would likely be limited. An implant can fill mild looseness, but using a larger or higher profile implant mainly to take up excess skin can make the breast heavier, increase bottoming out or recurrent sagging, and still not give stable medial fullness. You may need a lift, or a lift with an implant, depending on your exam and measurements. A board-certified plastic surgeon can assess whether implant dimensions, pocket control, and mastopexy would better match your goals.
