Question

Only desire is to address asymmetry and hide scarring rather than size. I will have internal scarring from: 1x original implants, 1x removal w/o replacement, 1x further implants, 1x en bloc capsulectomy and mastopexy (removed due to disproportionate aesthetic results, and development of an AI condition), 1x further scar revision. Would re-implanting necessarily lead to further AI issues, and would satisfactory aesthetic result even be possible in view of previous surgeries and internal scarring?

Thanks for your question. If you have had multiple previous issues with implants then I would suggest avoiding any further implant based surgery. Fat transfer (lipomodelling) may be a good option for you - this is where fat is harvested from another part of you (typically abdomen, loins, thighs), and is used to augment the breast. It can be performed on a daycase basis under general anaesthetic. You may require more than 1 fat grafting procedure, depending upon the result that you are aiming to achieve.

The challenges for the new procedure depend on the current shape and degree of scarring. An accurate medical history and physical examination will be the only way to determine if surgery has a good chance of success.

Given your history with implants I'd recommend not going that route again. Fat grafting could improve the volume asymmetry vs. reducing the larger one. There are options - I would seek out a few in person consultations with board certified plastic surgeons to see what the best options would be.

Re-implantation can cause additional complications. From your photograph, it appears your breasts are widely separated and you have contour deformity. You would benefit from scar revision and you may also want to consider fat transfers as an alternative to implants.

Revision surgeries carry a higher risk of complications than the original.  That is not to say you can't have a great result after a revision because you can.  But it is important to discuss clearly with your surgeon your past surgeries and what you are hoping to accomplish with the one you are planning. Depending on what the AI condition was that you developed following your implants, and the size you want to achieve, other non implant options could be possible (depending on examination) such as fat transfer to address asymmetry

Hi, thanks for your question.  something to consider could be fat transfer to breast(s) as a way of enlarging, addressing asymmetry and avoiding further implants which may (or may not) be related to previous AI condition.  This has nice spin off of reducing /reshaping another area eg tum / thighs - to harvest fat from and then this is placed into the breasts.  The principle downside of ftx is you can only increase by approx 1 cup size and it is difficult to predict exact size and outcome because some of the transferred fat is reabsorbed.

I would suggest first lifting both and reducing the larger breast to match. If you are not too scarred, then later consider small implants behind the muscles or possibly fat.
