You are in a grey area. You could have implants alone if your objective is to be larger but not really lifted. Implants are great for volume, not so much for lift. You could have a lift alone (vertical is best) if your objective is to be lifted and you don't mind being a little smaller (a successful lift removes some breast tissue from the lower poles). Commonly, I do a combination of implants and a lift in this situation to get the best of both worlds - a successful lift and also fullness of the upper poles. If you elect to have a lift, the vertical method is by far your best option and I discuss the reasons on my website.
Are your nipples located below your breast fold? It is difficult to determine this from your side photo but from the front, it appears that they may be. For this reason, a lift may be recommended.
If a breast augmentation alone would give you the shape and contour of your breasts that you wish, then you should just do the breast augmentation only. From your photographs, it appears that your nipple/areolar complex lies slightly low, and additional weight from a breast implant will likely push that slightly lower still. If this is acceptable to you, then the breast augmentation alone would be the operation you should choose. If you feel that your nipple/areolar complex is too low, and you wish it elevated to give you a more youthful and perky appearance, then you would require a lift. The trade-off of that you would have incision lines on the front of your breast.
Thank you for posting your question here!I will start by stating hat my answer is based on the limited information and pictures provided, for a more in depth answer and a bespoke plan I would recommend consulting in person with your plastic surgeon.An implant and a lift address different issues , Implants help add volume while a lift repositions the nipple and removes excess tissue/skin that is causing a droop. They can also be combined to achieve a synergistic effect in individuals with a both issues.That being said it is hard to assess the position of your nipples and droopiness based on the pictures provided alone, but if your nipples are in an ideal spot then all you need is an implant if increased volume is what you are seeking. You can always have a lift later if you are not satisfied with your final outcome. That being said don't get pressured into getting massive implants to compensate for any skin laxity, choose something that works with your frame and goals and if a lift is needed a massive implant is not a substitute for it , it will cause issues down the line. Implants alone will fail. I recommend a breast lift using The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained. I recommend an in person consultation with a plastic surgeon to discuss your goals and be measured. After I measure and discuss goals with my patients, we then look at pictures of women (my patients) with similar measurements with different size and style breast implants. They also get to see what they would look like when the breasts have fully dropped and fluffed and in clothes. You get to see actual results and the surgeon's experience and not a virtual idea of what can be achieved. There are some questions that need to be answered to give you the best recommendation. If you are looking to add fullness but are okay with the current nipple position then an augmentation alone will be sufficient. If you are looking for a perky look and would prefer that your nipples sat higher on the chest wall, then a lift would help you accomplish this.
Thank you for your question and sending your pictures. You do have breast ptosis and a lift would be needed to bring your nipple higher and tighten your lower breast. It all depends on how happy you are with your cup size, if you do not mind being larger then an implant with a breast lift will give you a nice result

You need a lift. Without a lift you will still have sagging but larger breasts. The implants would be placed in front of the muscle without a lift and you will sag more with time. With a lift, the implants are placed behind the muscle for better coverage and support.
