Thank you for your question and photographs.  The first thing to think about is what size breasts do you want.  This will help guide surgical treatment options. If you would like to see an overall reduction in your breast size, a breast reduction will allow you to lift your breast tissue, reduce the size of your areola, reposition your nipples into a more youthful position, and achieve a closer symmetry between your right and left breasts.  If you want to maintain your current size, a breast lift will help elevate your breast, allow for an areola reduction, and will reposition your nipples.  If you go this route, you may want to consider an internal bra to help take the weight of your breast tissue away from your skin, while providing additional support to delay a recurrence of breast sagging.  The cleavage you have demonstrated in your photograph can not really be created through surgery.
