I currently have the most cohesive Inspira implants. I’m lean, have minimal breast tissue and still have bad rippling despite the cohesiveness. I will be getting a revision. I’ve been told the Boosts might be a good option, and give me the projection I want due to the high cohesiveness. However I’ve read that the Xtras, although not as cohesive, have a higher fill level which might ripple less. So I’m confused as to what may be the better option to avoid rippling. Any thoughts please?
Answer: Implant position and fat transfers Unfortunately, you have not provided photographs or told us your bra size. If your implants are in the pre-pectoral position then you may be better off with them in the retro-pectoral position to minimize the rippling. Answer: Xtra are Too filled Mentor went too far with their Xtra product. That implant ripples too, because of too much overfill. A phenomenon most plastic surgeons have observed when injecting saline into saline implants, there is an inflexion point where side ripples have disappeared then begin to re-form while continually filling the device to greater volumes; Mentor's Xtra implants are right at that point. Although you don't provide enough information nor photos, I believe you are relying too much on the implant manufacturer/product and not on other parameters. You should really seek out some expert opinions in person.

Answer: Reduce Visible Implant Rippling Breast Implant related skin rippling is usually an undesirable feature and may result from several factors. A loose skin envelope relative to breast volume may contribute to rippling regardless of implant consistency. As you mention, the amount of natural breast gland over the implant impacts rippling, as does skin thickness, body fat, and implant position. There is a tendency for less upper breast visible rippling if the implant is positioned under the pectoralis major muscle. You also mention that more cohesive and more filled implants may ripple less, and this is certainly true. Capsular contracture creates more firm breasts, and tends to reduce visible rippling. Both Mentor BOOST and Mentor Xtra implants are similar in consistency to Abbvie Natrelle Inspira implants, so my opinion is that particular change alone will not significantly reduce your visible rippling. The reason(s) for your rippling must be analyzed and controlled to reduce visible rippling. The implant pocket size may need to be reduced to tighten your skin envelope, or perhaps surgical tightening of skin envelope by mastopexy if indicated and/or a larger implant (hard to know without photos of your situation). Covering the implant with Strattice, Alloderm, Galaflex, or another biological sheet usually reduces rippling, as does fat transfer to the breast skin in the area of rippling. An off the shelf filler (poly-L-lactic acid Sculptra) is another alternative. If rippling is in the upper inner breast when you lean forward, a push-up bra may reduce it sufficiently to delay or avoid surgery.
