I had bilateral capsulorrhaphy 7 months ago and while the result on my left breast is fantastic, I fear my right one is bottoming out again. I am wondering if from the pictures given, it seems like it will continue to deteriorate or if it seems stable - if the latter, I can be content, if the former, I would likely pursue a more substantial revision with more internal support.

It makes a little sense to add a new incision if you already have a scar from breast augmentation.
Going through the same incision will not make your current score. Look worse.

Since you needed a lift, keep your implants behind the muscle or you will sag fairly quickly. Also, you will have better upper breast fullness with implants behind the muscle. Bigger will sag sooner.
