I would recommend a secondary lift to achieve more symmetry and elimination of the excess skin in the vertical and horizontal dimensions. I would express your concerns to your original surgeon.

I think you would benefit from a revision of the left side which will significantly improve your asymmetry. What must be done is resection of the breast tissue and skin in the lower half of the breast, otherwise you will not benefit. Thank you.

The degree of asymmetry you had before surgery made this a very difficult procedure I think your result be improved. I think you need an implant on both sides. The problem with a lift with no implants is that it usually lacks upper fullness. It is unclear from your description if you have implants in both sides or one only

It’s very normal for patients to have some changes in results over time after breast lift surgery with or without implants.
When patients have asymmetry, then this can create increased asymmetry overtime.
Plastic surgeons try to anticipate changes that will happen overtime but doing this accurately every time is difficult.I think the reason your areola A smaller on the left side is because you had a lift done in conjunction, which really helps keep the areola small.
A unilateral augmentation is going to set up a patient for postoperative asymmetry.
Implants create breast projection that lift cannot accomplish.
Each plastic surgeon will have their own decision, judgment, and policies in regards to revision surgery.
To me if you’re going to do a revision then you may as well do everything you can to give the patient the best outcome.
What I do has nothing to do with how other plastic surgeons practice medicine.
It seems to me that a more extensive revision would be in your favor.
