I’m 45, 5’3 and 120lbs. Feel pretty fit and cute until I see my lower face. I’ve always had a round, flat face and with fillers I feel like it’s even fatter. They have now worn off and I’m wondering what to do now. Dr had recommended fillers last time and said I wasn’t ready for facelift. Although I don’t necessarily want to have to get surgery - I also want to prevent the aging and fix it as much as possible with best bang for my buck and money is not really a problem- recommendations??
Answer: A facelift is a great option for you Hi there, First of all, you are very beautiful! I tell my patients that typically after the age of 40, the best "bang for your buck" is surgery as fillers will no longer due the trick and often end up distorting your natural features. Rather than injecting large amounts of fillers to try lifting or camouflaging the jowls, which can cost thousands and appear very unnatural, a facelift surgery would simply reposition the tissues to where they once were and actually appears much more natural than all the things you might try in order to avoid surgery. I also perform facial fat grafting (replace like with like) with all facelifts and one of the areas that it makes the biggest difference is around the mouth/jowls. Hope this helps!

Answer: Face lift I agree you're pretty fit and cute! :) For your concerns, you absolutely need to consider a surgical procedure. Filler is great for some things but once you are looking "fatter" with filler, then it is time to stop and just go ahead and get the lift procedure. For you, I would recommend a full deep plane face and neck lift. If you are open to considering other things, I think you would do great with an upper lip lift, some facial fat grafting, and possibly an upper bleph too. Good luck!

Answer: Surgery to improve jowls Facelift and neck lift surgery does a wonderful job targeting the lower face and neck for reversible signs of aging. Some of these signs are present in your photographs. Delaying surgery as long as possible is always a good idea, however it sounds as though you are ready for more definitive treatment than can be offered with non surgical modalities. A good face/neck lift surgery will improve your jaw line definition significantly, treat the jowls, and provide more definition to the neck. Depending on your goals, and concerns, you can consider other treatments for the skin and other regions of the face. Unfortunately surgery does not prevent further aging as time marches on. It would be very reasonable to seek a consultation to discuss your options with a board certified plastic surgeon at this stage.

Answer: Mini lift with vertical SMAS plication would be an excellent option. In order to smooth your jowl, and straighten your jawline, the fascia layer of your face, the SMAS, would need to be elevated in a vertical fashion, to elevate the jowl off of your jawline and efface it. This could be done through a mini facelift. A simple skin lift would likely not have long lasting results as the skin is not a support structure. It is a covering. Asking that skin to be a support structure leads to early recurrence of the jowls. Best wishes.
