Upper reverse tummy tuck in combo with a lower mini tummy tuck. Any risks or negative aspects of this? (Photo)

First, your belly button looks good in the photos, and my guess is that your skin tone would not allow a full abdominoplasty to be done without a high scar. Depending on what your skin tone is, a mini could work nicely for the lower abdomen. The reverse tummy tuck would place the scar where the bottom part of the breast lift scar would be except for the part across the midline. You might be able to get a little bit of lift with excision of some skin only below the breasts, but this would be minimal. It is hard to second-guess the surgeons who have actually examined you personally. See my website for pictures of the combo you describe ("bidirectional abdominoplasty"). It is not a common procedure but it might be a reasonable option for you.

Thank you for your question and posting an appropriate picture. There are a few patients that are good candidates for a mini abdominoplasty. Based on your picture, you seem to be one. Your bellybutton area of concern can be corrected during a mini tummy tuck with endoscopic plication up to your xiphoid process (breast bone). You need to be examined in person for precise  recommendations.

From the photographs you look like an excellent candidate for a more traditional abdominoplasty were the incision is below the bikini line horizontally across the lower abdomen. I've never heard of the combination of procedures you just described.

Personally I would not recommend this.  The incision required is NOT the same as the one for your planned mastopexy and will also lead to forces opposing the healing of your planned "mini-tummy tuck."  I would suggest seeing more consultations before committing to this surgery.  In the vast majority of patients, there is plenty of skin to perform the traditional procedure.Good Luck.

Reverse TT can leave a very unsightly scar with minimal cosmetic gain. I would caution on combining both procedures together as they will be working in opposite directions of pull.  Best of Luck.

You have an interesting dilemma.  I would worry about healing when combining two opposing forces. If you have had previous pregnancy then there is almost always enough skin for a traditional tummy tuck especially with some liposuction loosening the flanks and tightening the muscles. NEVER have heard of a mini TT with a reverse TT. BAD IDEA would most likely end with necrosis of the skin flaps! 

You should stick to a low transverse bikini type incision with repair of your anterior abdominal wall.  A reverse abdominoplasty is rarely used and the resulting scar is difficult to hide.  You may need a small lower vertical inverted T type scar if it there is not enough skin to complete the standard abdominoplasty.

Based on your pictures alone, you may be a good candidate for liposuction alone with an umbilicoplasty. (belly button surgery). IMO, you don't need a full abdominoplasty. A reverse abdominoplasty will not satisfy your belly button concerns and may result in a noticeable scar.Best wishes!
