Platysmaplasty or Neck lift - which option is best for addressing my platysma bands and weak chin?

Hello, I'm almost 33 year old and suffering from terrible platysma bands and laxity under my chin. I am thin and workout a lot. However have always been insecure over my weak jaw and chin. I have tried Botox multiple times without improvement. I am wondering if platysmaplasty or neck lift would be best. Currently seeking consults but haven't got any response.

I would recommend a deep plane neck lift to sculpt the structures below the platysma muscle and release the platysmal bands (also perform a platysmaplasty). This will help reduce the volume below the jawline and provide better contour. You need to cut the bands directly and release the posterior cervical fascia (this is where the platysma muscle inserts into the neck and causes the linear bands). Despite this, there is still a chance of platysmal band recurrence (up to 50%) chance. You need to find an American Board of Plastic Surgery certified surgeon; ideally, this surgeon has also completed an aesthetic fellowship accredited by the Aesthetic Society.

Based on your photos and your age I would start with some Botox for the bands and maybe RF microneedling for the skin laxity. If your chin bothers you then a chin implant works very well to add projection, and this also help with the appearance of skin laxity under your chin and jaw line. A surgical neck lift would be a bit too much for your concerns! Hope that helps!

Thanks for your question. Looking at the photos you've provided, I have a sneaky suspicion that your best option would a proper necklift with platysmaplasty and placement of a subtle chin implant to vertically lengthen and project your somewhat turned up chin on profile view. When expertly performed, this will create better jawline shadows and give your face more balance than if you were to perform a platysmaplasty alone or platysmaplasty with chin implant. The patient in my link below has had this exact procedure.

You are young and neck looks great. I would recommend platysmaplasty and thermitight combination. This will remove the bands and tighten the skin.

It does not look that platysmaplasty/neck lift would do much for your platysma bands. They will be visible to  some degree even if you have very aggressive platysma muscle resection as a part of platysmaplasty. Not sure about chin, it looks appropriate? You should start with Botox injection into platysma bands for time being. Good luck. .

A platysmaplasty is simply the medical word for a neck lift, not necessarily a separate procedure. A platysmaplasty addresses the muscle that drapes the neck. Some patient's have separation of this muscle which can leading to banding. Neck lifting can be addressed via either via an isolated incision under the chin (submental incision) or in combination with incisions that hide behind the ear. the muscle separation is repaired to improve banding and contour of the neck. The need for incisions behind the ear is based on your individual anatomy and should be a decision you make with your surgeon after examination.

Please post a full set of facial photographs of your face, chin, and neck from all angles to make a determination about how best to proceed. A neck lift procedure is performed when there's an excess fat deposits above and below the plasma muscle in your neck, which also includes a platysma plasty to significantly improve the jawline. Prominent plasma bands can be addressed with simple Botox performed in the office setting. When there's a recessive chin present, then consider placement of a chin implant.
