Your nose would benefit from shortening and narrowing and this could be accomplished with a standard closed rhinoplasty. Also, consider alar wedge resection to reduced the flaring of your nostrils. I get the impression you feel this would be a complicated rhinoplasty, but it really is pretty basic. Just make sure you get multiple opinions and choose your surgeon carefully. You shouldn’t need anything fancy like cartilage grafts but I’m sure surgeons will recommend them.

A tip plasty can effectively reduce this. In some cases a full rhinoplasty might be needed. Your nose is on the large side and a reduction rhinoplasty might be needed.

Reduction rhinoplasty is the term used for making a big nose smaller. However it can have many components.

2. Large as in big tip
3. Wide nostrils

Hello and thanks for your question. Based on your photos you need rhinoplasty. You have a nasal hump, long and wide nose and bulbous nose tip. Wideness of your nose can be decreased with ultrasonic rhinoplasty( ultrasonic device is used to reshape the bone without breaking the bone). Nose hump can be also removed with ultrasonic rhinoplasty. Length of your nose can be decreased  and the tip part of your nose can be reshaped to get more defined, feminine nose tip. . This combination will give a good result for your case.

Thank you for your question. A large portion of my practice is dedicated to rhinoplasty. You are an excellent candidate for rhinoplasty. I think it is one of the most beautifying surgeries that we do as plastic surgeons. In your case, removing the caudal (bottom) portion of the septum and shortening your nose should elongate your upper lip. You should be able to have a very attractive nose. It is important to only see plastic surgeons with extensive experience in rhinoplasty, ie perform at least 6 per month and have performed hundreds of cases. Also, critically evaluate their on-line galleries to see if their art form resonates with yours. Rhinoplasty is definitely a grey haired plastic surgeon's sport. Best wishes to you

The good news is that there is the potential to markedly improve the tip by elevating is upwards (rotating) and the securing it in the correct position. There are lots of ways to do this and it may involve attaching the tip onto a secure cartilage foundation to avoid it dropping again in the future. There is also a muscle going from the upper lip into the tip which can be divided to avoid the downward pull on the tip as you smile. This would in effect, lengthen the upper lip somewhat to enhance the overall appearance.

A closed rhinoplasty approach can accomplish shaving down the dorsal hump, narrowing the bridge line, lifting and refining the nasal tip with all the incisions placed on the inside of the nose. You are a great candidate for a rhinoplasty. Your nose is too long and as a result it hides much of your upper lip. It is even more obvious when you smile. Part of it is that you have a relatively short upper lip but a good rhinoplasty will shorten your nose by rotating your tip up enough so that the angle that you nose makes with your upper lips is more attractive. The ideal angle is just over 90 degrees. Yours is LESS than 90 degrees. Patients are always worried about their noses getting too turned up, which is a reasonable concern, but it is also important not to have your nose too long.

The most definitive way to improve this would be a rhinoplasty to improve the position of the tip cartilages at rest and when you smile.

You could try a small dose of Botox to the muscle (depressor septi) which makes the nose drop when you smile. This may help very slightly for about 3 months.
