47 yr old F 110 lbs now 7 yrs post revision chin implant and still hate it. Ever since surgery you can see there is a crease in my face on the right profile, and there Is a very prominent horizontal indentation across my chin. The skin on my chin look puckered i think from nerve damage. I can’t believe I went in for rhinoplasty and the surgeon convinced me to redo the old gortex chin implant I had with this silicon wing implant and botched my chin like this. My kids now ask why my chin looks so weird. It makes me very sad. I’m afraid of surgery after this but obviously it’s the only way to fix this. Would you recommend sliding genioplasty or face lift? Should it be a deep plane facelift or doesnt have to be? Open to how I can get rid of the creases on my chin and get rid of the double chin appearance I now have.

The photos show a horizontal chin crease and contour irregularity near the implant area, along with some submental and neck soft-tissue laxity. Puckering in a photograph does not prove nerve damage; implant position, capsule, soft-tissue tethering, and mentalis muscle function all need to be examined.

A sliding genioplasty changes the position and shape of the chin bone. It can be useful when the underlying bony projection or vertical height is inadequate, but it will not automatically release a scarred implant pocket or correct neck laxity. A facelift or neck lift addresses lower-face and neck tissues, but it does not directly correct an implant-related crease. A deep-plane facelift is therefore not the default answer solely because the chin looks irregular.

The next step should be a revision consultation with a board-certified facial plastic or plastic surgeon experienced in chin implant complications and osseous genioplasty. Bring both operative reports and implant details. Examination, standardized photographs, and often CT or cone-beam imaging can assess implant position, bone changes, and chin anatomy. Depending on the findings, treatment may involve implant removal or exchange, capsule and scar release, mentalis repair or resuspension, selective fat grafting, sliding genioplasty, and a separate neck procedure if needed. Planning the chin and neck as two related but distinct problems is safer than choosing a facelift label first.
