Based on the photos and your description of a transient change in the shape of the nipple when the nipple is hard is usually indicative of the presence of breast tissue directly behind the nipple areola complex. When the nipple is hard (usually due to cold exposure) the chest looks better because the contraction of the muscle within the nipple areolar complex flattens out the breast tissue behind the nipple areolar complex. When the weather is warm, the nipple areola complex relaxes and the breast tissue is more prominent often prolapsing into the nipple areolar complex. Anabolic steroids can contribute to the presence of male breast tissue. The presence of breast tissue requires an excisional procedure to correct the issue. We use a minimally invasive approach under local anesthesia which is approached through a 5 mm incision in the anterior axillary line.
It is likely glandular tissue directly under the nipple. This will in all likelihood need a direct excision and not lipo.
The answer to your question is that the excess glandular tissue that is causing the protrusion needs to be removed. There is no type of liposuction that effectively removes glandular tissue.
Thank you for your question and photographs. From what I can see, you have true glandular hypertrophy or extra tissue that is breast like underneath the nipple. No type of liposuction is able to remove this tissue. In my experience, a small incision around the areola is necessary to reomve this extra tissue with an open approach. I would recommend you see a board certified plastic surgeon with demonstrated experience in cosmetic male breast surgery. Best of luck.
