I'm 49 and considering a lower facelift. I would like to lift the jowling I have and tighten the skin under my neck. Thank you.

You are a very good candidate for lower face/neck lift; it will shape your neck by means of removal of fat excess, thighten/shape platysma muscle, address the jawline and jowls. In person consultation with a plastic surgeon with good reputation in face/neck lifts, is recommended. Good luck.

To control better the laxity of the skin under the chin, a more vertical vector for the facelift would be valuable. The deep plane approach for facelift tends to provide this type of impact and it seems like a favourable solution for you.

For the neck contouring, there is also some volume excess which would not be corrected by the skin tightening alone. The deep plain necklift intervention can address the neck girth more effectively and produces an improved balance between the characteristics of the face and of the neck.

You would do great with a lower  face/neck lift under  local with a little iv sedation for complete comfort. Downtime is 1-2 weeks and there is minimal to no  pain afterwards. Bruising is minimal and you look amazing in only 2 days.

From the photos, I would recommend lower facelift ( will correct the jowls ) and lift up the cheek (as long as it is a deep plane facelift ). I also would recommend a neck lift to make sure your lower face and neck are balanced and the transition is smooth and youthful.

The short answer is a neck and lower facelift may not be enough. Even though you are young, the heaviness in the neck and lower face may require more advanced techniques, such as deep plane facelift and deep neck lift, to achieve a satisfactory result. The corners of the mouth are typically not lifting (at all) with traditional SMAS lower facelift and require an extended deep plane approach in which the entire midface and midfacial fat pads are mobilized and key retaining ligaments of the face are released. Without the midface dissection and release of retaining ligaments, the corners of the mouth and cheeks will not be mobile enough to be repositioned enough to improve those areas. Consider consulting with a surgeon that performs extended deep plane facelifting.

Hello, hope you are well. Based on the photos you have provided, I think you would be pleased with the results of a well-executed face and neck lift. There are some individual characteristics that can limit your results such as the shape of the jaw and position of the hyoid bone, but you appear to have sufficient laxity and soft tissue redundancy in the lower third of your face and neck to warrant a surgical approach. The corners of the mouth/lips are not 'lifted' per se, but the changes that take place peri-orally by reducing the jowls and soft tissue immediately lateral to the mouth can create that illusion. A non-surgical approach to consider would be radiofrequency treatment called FaceTite +/- Morpheus8 which can tighten the skin and reduce subcutaneous fat to a degree without the need for long incisions or general anesthesia. However, the results are more limited. Meet with a plastic surgeon who specializes in surgery of the face and neck to better understand your options and results that can be expected.

Based on your photos, I would say that you appear to be a good candidate for a facelift.  This procedure tightens the skin of the lower face and neck.  I would also recommend liposuction under your chin at the same time to improve the fullness under your chin.

You might wish to consider HA fillers before jumping into expensive, aggressive, surgeries that can entail significant post-surgical downtime. The profile shows considerable volume deficit in the chin and lack of projection. An appropriate HA filler for augmenting the chin can increase chin projection and create greater harmony between the projections of the chin, lips and nose. This would also serve to tighten and better define the jawline, which, in turn, can also serve to pull up the skin under the chin. A second HA filler can be used to lift and straighten the corner angles of the mouth and reduce the marionette lines. All of the above can be accomplished, if so desired, in one treatment session, which, in the hands of an experienced, board certified cosmetic dermatology injector can be done in no more than twenty minutes with immediate, i.e. on the spot, results.
