Is it possible to create a more rounded hairline through a forehead reduction?

I have a very square hairline along with a large forehead. I have heard that a forehead reduction can lower the hairline but that hair transplants would be needed as well to round the hairline out. Is it possible to round out the hairline with just a forehead reduction (by taking the forehead down/in more on the sides)?

I find that we cannot reliably make changes to the hairline shape with forehead reduction surgery, but we can with hair transplants. For these reasons, if the patient's primary goal is hairline shape changes, then we may lean more toward transplants. If the patient's primary goal is reducing the size of the forehead, then I lean toward forehead reduction surgery. If a patient wants to attack BOTH of these issues, then they need to understand that a multi-stage procedure is going to be needed, and this will very likely involve a forehead reduction surgery followed by a hair transplant procedure to round out corners or alter the shape slightly.Be sure to see an expert who is well-versed in all of these options and procedures.Best,

Hair transplant surgery with individual hair follicles are better suited to ROUND OUT the hairline.  Forehead reduction will be limited in rounding out the corners.

About 15 percent of my patients have hair transplants approximately 3 months after a hairline lowering if they desire a more rounded hairline. This is especially true in females with temporal recessions.  I find this to give a more natural result than just lowering a female hairline with hair transplants.

Depending of the type and degree of forehead and hairline reduction, different techniques may be employed. Reduction of the forehead bone has limitations (without major osteotomies) but in combination with a hairline manipulation it can create significant improvement. Facial feminization with hairline lowering and female, rounded hairline. More than 40 years ago I developed the facial feminization technique where the hairline is lowered as well as partially rounded. A small number of micrografts are the necessary to create a softer rounded female hairline. I feel this is still the “Gold Standard” for giving the best results and least expensive way to do this. Forehead reduction surgery (hairline lowering surgery) is a type of plastic surgery that creates a smaller forehead. The actual shape of the forehead is not changed, but the hairline is lowered so the forehead looks smaller. You also won't need hair transplant for this surgery. 
   It is suitable for most forehead shapes because it allows custom designs of the hairline to balance it with facial contour. In your case angular shaped forehead reduction is proceeded through diamond-shape hairline lowering. An incision is made at the hairline and depending on the scalp laxity, 2-3 centimeters of excess skin can be removed. Endotine is used to fixate the scalp and the remaining parts are stitched together.

In the forehead reduction/hairline advancement surgery, the incision is designed such that some rounding of the hairline is achieved.  However, in most cases, if a significant narrowing of the width of the forehead- i.e. a much more rounded look  is desired, hair grafting as soon as 3 months later is to be performed.

In some situations a more rounded hairline can be achieved with a forehead reduction procedure but this will depend on several physical characteristics. How much do you want to lower the hairline and how loose is the scalp because the fronto-temporal junction (lateral hairline)must be lowered more than the central hairline. An expander may be necessary. How thick is the hair at the end of the existing hairline? Ideally, the rounded part of the hairline is softer and less dense. Pictures would be helpful.
