I wish to have a second round BBL, however the doctors told me I could not have another session unless I gain more weight. I’m currently 65kg, 172cm tall (BMI 22) and have already had aggressive liposuction in all my problem areas. Because I had to gain a lot of weight for the initial surgery (that I plan on loosing eventually), gaining even more for the second round is not an option for me. Could I find a surgeon who would perform a second round on me in my current weight?

You are in good company with many others who had high hopes for BBL but instead received disappointing results (lumpiness, unevenness, lack of projection in the central and/or lower buttock, post-deflation sagging, etc.) as most of the fat melts away. It is not quite as much the quality of the doctor nor liposuction technique but rather the quality of the procedure that is a large part of the problem. Thus, going back for round 2 BBL most often produces the same disappointing results but at another financial loss. Also, beware if a surgeon asks you to purposely gain weight (i.e. fat) before BBL, know that the fat you lose first as you get back to your baseline weight after surgery is that very same fat that was transferred into your buttock, hence your new buttock shrinks first, the fastest, and the most...so please do not fall victim to this recommendation. Buttock implants indeed provide a more reliable, predictable, relatively safe, and permanent result, especially for adding projection to the buttock and often smoothing out lumpiness. Occasionally a lower buttock tuck may be advantageous for lower buttock fold asymmetries and/or sagging. Please keep in mind that an extremely few number of board-certified plastic surgeons specialize in buttock implants so be sure to consult with one that is reputable and experienced. Unfortunately you may have to travel long and far away from home to find this particular surgeon. Glad to help.

A Skype or FaceTime consultation with a BBL specialist can help you get an accurate recommendation. If you are a good candidate (less than 5L of fat) for a Brazilian Butt Lift (BBL), the optimal outcomes are achieved with: 1) use of VASER liposuction to harvest the fat, 2) a closed looped system such as Puregraft to process the fat, and 3) strategic micro-packet transplant technique. These three processes have demonstrated over 95% fat graft take. This means that your results will be stable as long as you maintain your weight. As an official VASER trainer, I am routinely referred unhappy BBL patients. One of the most common reasons for disgruntled BBL patients is that patients are not able to accurately communicate their desired buttocks goals to their surgeons. A buttocks assessment tool can be an invaluable tool to communicate to your surgeon the exact buttocks size and shape you are desiring.

In my experience assessing individual lobby in my patients or patients who have had previous liposuction always requires an in person consultation.  I do not recommend anybody gain weight for fat transfer procedures. If you lose the weight in the future it makes the whole process pointless.

Fat needs to be harvested from multiple areas this should include the entire torso including under arms and full back as a general first place. if that is insufficient then patients should consider including arms and inner hands possibly anterior(front)  thighs.

For some people having a high-volume fed transfer procedure is simply not feasible and should not be attempted.
