Buttock “sagging” is usually a combination of loss of volume or deflation and/or the effect of aging and gravity. So, I personally first treat patients like you by restoring volume to the buttock with a buttock implant (BBL is a clearly not an option for you and bound for 100% failure, period). This will create a more desirable smooth and rounded perky buttock profile. If not adding volume with an implant, the buttock will be shortened but may appear more flat with just a buttock lift. A lower buttock tuck is usually then performed at six months following implants. In this sequence, the implants have fully settled into final position so that any remaining loose skin can be most accurately identified and removed with the lower buttock tuck. Note, Skin tightening devices such as J-plasma, ultrasound, laser, radiofrequency will all fail in your case. Please note, because an extremely few number of board certified plastic surgeons specialize in buttock implants, you may have to travel away from your local area to seek consultation with this particular type of surgeon. I do many Sculptra revisions to buttocks, but after a "skinny BBL' the results are 50 vs 50% improvements. Especially in inferior lose of definition in skin inferior gluteal folds. Your post does not make it clear of what you had done or what your concern is.

I BBL is generally the volume increasing procedure not one that creates a lack of volume.

If you need volume fat transfer is the best option and if you need volume removed Liposuction is the way to do that.

To find a provider I suggest you start scheduling a series of consultations with local board-certified plastic surgeons who seem to have a knack at both Liposuction and fat transfer procedures.
