Is it mandated by law to keep a record of the serial number of the Juvederm injection at a patient medical record?

About 4 month ago, i got a syringe juvederm injection on my nasolabial area and cheek. I chose a board certified dermatologist in new York , but after that injection I have neurology problems as well as circulation problems. I got disappointed when I asked for my medical record, and they told me that sometimes they do not keep record. I can't report my side effects to allergen without a serial number it is not valid my complaint. Are they breaking a law??

we have a certified OR in our office and as per AAAHC policies we are required to keep a log of every medication thats administered to our patients in the office. and that includes Fillers, Botox, kenalog, antibiotics etc

Thanks for sharing your excellent question and I am sorry to hear of your neurological and circulation related issues.  Each state varies in their requirements but for most physicians medical records they need to be available for 7 years.  I do list the lot number and expiration date of each filler, neuromodulator, or other injectable for record keeping purposes. Hope this helps.

Thanks for reaching out.  I'm not sure if it is illegal.  Likely that varies from state to state.  We always keep a record of the serial number and exp date of all products used.I hope this helps :)

Juvederm syringes all come with a lot number and it is standard practice to put the "sticker" that comes with that lot as part of the patient's records.  You can check with Allergan as to what is "legal" or not.  Please consult an expert in Juvederm and facial injections for the best cosmetic results.
