Dear evez2072,my preferred lift is a lollipop lift when a periareolar won't do (ie greater than 2cm lift required). The lollipop technique was created by a Canadian plastic surgeon named Elizabeth Hall-Findley and treats the breast as a three dimensional structure instead of two dimensions (anchor - lift). Most importantly it does not require the horizontal component scars that the anchor-lift requires and breasts appear much more natural, lifted, and less boxy. Lastly, the lift lasts longer because the lollipop doesn't rely on the skin to hold the breast up, its actually breast tissue thats being brought together to support the breast and hence improves longevity. I recommend an in person consultation with a plastic surgeon to discuss your goals and be measured. A breast lift will remove excess skin and raise the nipple position. You can consider a small reduction if you would like smaller breasts. Another option for you to consider is auto augmentation. This is an approach that uses your own tissue to create more upper pole fullness. This is not a fat transfer. I've included a video that I hope you find helpful.
A lift would do well for you. You do have plenty of breast tissue and don't need an implant for more volume.
Thank you for your question. Based on your pictures, I would suggest a breast lift. Depending on you goal size after surgery, you may also be a candidate for what we call an "auto-augmentation". This is where we use some of the tissue from the bottom of the breast that would normally be removed during a lift and sew it up higher on the chest to create a little more fullness centrally. When combined with a skin excision (likely anchor lift), this can re-position your breast tissue higher on your chest as you are showing in your "after" picture.
