I CANNOT speak highly enough about Dr. Lopez. Professional, knowledgeable, caring, kind, great communication, accessible, and most importantly TALENTED! Absolutely love my results! I'll start by saying that the initial consult was so informative and Dr. Lopez addressed all my questions and concerns thoroughly with kindness and respect. Unlike other consults that I had. I booked right away because he made me feel so comfortable. My initial surgery went SO smooth. Didn't even need pain meds afterwards. However I was an idiot and my left breast got wacked good 3 weeks later and I got a large hematoma. That happened on a Friday night and I right away phoned DR. Lopez. He talked me through the whole thing and after checking on me again shortly after he realized that I would need surgery to drain it-and he booked mine and my husbands flight THAT night and paid for them himself and did the surgery at no cost that Monday. He even covered the surgery fees! All out of the kindness of his heart because he really cares and wanted me to get good results. We are seriously blown away by Dr. Lopez skill and kindness. And he is always ready to reply to any concerns that I had all along the way. I cannot recommend him enough!!
I am a travel patient with Dr Lopez @ Amelia Aesthetics of San Antonio all the way from NC! My surgery was back in late February 2024. I am also a local patient of Amelia Aesthetics of Raleigh. My surgery with Dr Lopez involved a second extensive lift along with a breast augmentation. Not only was the surgi-center professional, clean, well organized and overall amazing, I feel that I got to know my surgeon on a personal level via zoom meetings and at my pre-op appointment the day before surgery. This office is beautiful! I already had a pretty decent idea of profile/sizing before I arrived to TX, but we took photos, weighed sizing option ranges once more, and reviewed expected outcomes/goals to make sure we were on the same page. I will never forget Dr Lopez having me verbally confirm exactly what I was going under for right before they wheeled me to the operating room. It was nice knowing that he was also making me accountable for my final decision. Recovery was a breeze! If you are looking for an amazing plastic surgeon, this is the one. I flew in from out of state to have my implant exchange done and could not be happier. From the moment I called to schedule a consultation I have felt truly taken care of and important. Dr. Lopez takes his time explaining the procedure, your goals and what to expect after the procedure. His bedside manner is second to none. He called me the evening of my procedure from his cell phone to see how I was feeling and then again the next late morning to check on me again. I would not hesitate to recommend him to anyone who will listen. I will be returning in December to have a tummy tuck.
Some surgeons were trained to do these two procedures separately and that is what they recommend. Sometimes it has to do with your tissue and how they feel it will be able to support an implant. Another reason could be the size of the implants you are hoping to have placed. All that being said, in my practice, I commonly perform the mastopexy at the same time as implants and I only separate the surgeries for a few patient specific reasons.
This is a great question. You are right that subfascial gives a little more control when it comes to trying to decrease the gap between your breasts. It also tends to have less movement when you are using your chest muscles. The downsides of over the muscle include an increased risk of capsular contracture as well as a risk of seeing the edge of the implant or rippling. The type of implant you choose can play a role in that risk as well. Both surgeries have their benefits and downsides so it just depends on which one fits with your goals and lifestyle best.
Thanks for this question. The textbook tells us that if you can pinch 2 cm of tissue in the upper pole of your breast, you have enough for OTM. I think of it a little more globally. The risks of going OTM include a higher capsular contracture rate (not related to amount of tissue) and the risks of being able to see the edge or ripples in the implant. I ask my patients to weigh the risk of rippling (this bothers different people to different degrees) with the benefit of not having to go under the muscle. I think your pictures appear to have enough tissue to go over the muscle and the discussion with your surgeon should center around giving you all the pros and cons so you can make a decision that feels right for you.
Thank you for your question. Picking the right implant can be an intimidating and stress provoking process. Sizers are helpful but remember they aren't a perfect estimate for what an implant will do once it in in your body. 3D imaging software like Vectra can also be helpful to allow you to see the implants simulated on your frame (I use a combination of sizers, Vectra and wish pictures). 30 ccs sounds like a big difference but it is actually really subtle once you factor in the way it interacts with your tissue. It is about 2 tablespoons worth of volume (some people find it helpful to put two tablespoons in a sandwich bag to see how small of a difference it is). Hope this helps!
Thank you for the question. It is hard to tell for sure without an exam but I do think you appear to have enough tissue to have over the muscle implants. There are plusses and minuses to each technique but the one that relates to your upper pole tissue is the chance that you will see rippling. You can have visible rippling with either technique but it is higher with over the muscle implants. This is where a conversation with a plastic surgeon about technique and implant choice can really come into play.
