Is a breast lift needed? (Photo)

Hi, thank you for sharing your question. The ideal procedure for you will involve a lift because your nipple position is very low. Implants alone will not give you an aesthetic result.

I would strongly recommend a breast lift if you are considering breast implants. The lift will help the shape and position of the nipples in relation to the implants. Implants will just make your breast bigger but they will still be saggy.
I've had plenty of patients who had keloids from earrings or other smaller procedures heal just fine from breast lifts. Scar gels, tapes, steroid injections can help the scars heal as smooth as possible.

In your case , I definitely recommend you lift as well. A breast lift surgery performed in conjunction with breast augmentation is a terrific way to create a fuller, perkier breast. A breast lift will address the sagging skin for a more youthful look. Combined with implants, you will achieve increased volume and resolve sagging issues as well.

In person exam is essential. Based on limited photos - it appears that you would benefit from a mastopexy/ breast lift with or without small implants . You can only achieve perky breasts with a lift. I suggest that you seek an experienced Board Certified PS and review all options at your consultation

Although the implant will fill the flat upper pole, the breast will appear to "fall off" the implant giving the so called "waterfall deformity." While some people may not mind this look, the vast majority of women would consider it aesthetically displeasing. This, of course, must be weighed in conjunction with your tendency to keloid scar, but the preference of most surgeons is likely to be to perform a lift and aggressive keloid management post op.

based on your photos, it looks like you would need breast lift. my preferred lift is a lollipop lift when a periareolar won't do (ie greater than 2cm lift required). The lollipop technique was created by a Canadian plastic surgeon named Elizabeth Hall-Findley and treats the breast as a three dimensional structure instead of two dimensions (anchor - lift). Most importantly it does not require the horizontal component scars that the anchor-lift requires and breasts appear much more natural, lifted, and less boxy. Lastly, the lift lasts longer because the lollipop doesn't rely on the skin to hold the breast up, its actually breast tissue thats being brought together to support the breast and hence improves longevity.

Your two comments in your paragraph are that you wanted the empty space filled in your breast and ideally you would like to have bigger breasts without the lift. As long as you realize that you have a grade 2 and that you will end up with a bigger version of your grade 2, then a breast augmentation will increase the volume in your breast and it will leave you with a 5 cm incision in the fold underneath your breast and nowhere else. This meets your criteria of filling the volume in your breast, avoiding a lift and minimizing your scarring. Have a detailed consultation with a plastic surgeon. Best wishes.
