M659 Cc moderate plus silicone under the muscle. Planning revision in a few months. How can I get them closer together?? They fall into my sides when I lay down too! Totally too spread apart. Low on my chest as well!

To achieve more cleavage the implant pocket is designed more medially and great care is taken to avoid dissecting laterally.

Thank you for your question.  A large portion of my practice is dedicated to breast surgery and revision breast surgery.  I have been in practice for over 28 years.  In your case, I would perform an internal bra procedure - capsulorrhaphy as well as revision of your mastopexy.  I wrote the very first paper on capsulorrhaphy over 20 years ago.  Some patients require removal of inferior pole breast tissue well as the revision of mastopexy - breast lift.  This is termed reductive augmentation.  You should be able to have an excellent result. Your photograph is very limited. However, the technique I recommend is explantation and lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. Later, fat transfers or small implants can be placed if additional volume is desired.

Naturally, breast do fall to the sides when you lay down. However, your implants appear a little large and heavy for your tissue. You can ask to have your lateral and inferior pockets tightened so there won't be as much movement.

Unfortunately, your photograph does not reveal the lateral aspect of your breasts. The appearance of “a wide breast“ can often be caused by excess lateral inferior breast, extending into the axilla. This can be corrected but a better photograph and in person exam would be needed for confirmation.

To make an assessment on the outcome of a plastic surgery procedure we generally need to see a complete set of proper before and after pictures. If you don’t have before, and after pictures, then ask your surgeon to forward the pictures they took. The outcome of breast augmentation surgery is based on three variables. The first is the patient’s candidacy for the procedure. Not all patients having breast augmentation‘s are ideal candidate for this operation. The second variable is the choice of implants which includes size, shape, and type. The third variable is the technical aspect of placing the implant in the anatomically correct position. Women who have diverging breast are still going to have diverging breast even if they have implants placed women who have ptotic breast may need a mastopexy surgery to correct droopiness.
There isn’t much wiggle room in regards to where to place the implant. It needs to be centrally paced on your breast base on the chest wall. There is a correct place to put the implant. Any issues not related to implant placement need to be accepted as baseline or require different procedures. 
I suggest you follow up with your provider or schedule a few in person second opinion consultations.
Consider obtaining a complete proper set of before and after pictures from your provider.
