You can definitely benefit from a breast lift as your breast tissue has migrated down and is no longer over your implants. The purpose of breast lift is to create better symmetry of your breasts, improve the shape, and the nipple position as your one nipple is lower. You have the option of exchanging your implants as they are 13 years old and more likely to leak. In that case you will have the option of going smaller or stay the same as you said you don’t want to be bigger.

Based on these photos, you are a good candidate for a lift. I would not change the implants unless absolutely necessary.

my preferred lift is a lollipop lift when a periareolar won't do (ie greater than 2cm lift required). The lollipop technique was created by a Canadian plastic surgeon named Elizabeth Hall-Findley and treats the breast as a three dimensional structure instead of two dimensions (anchor - lift). Most importantly it does not require the horizontal component scars that the anchor-lift requires and breasts appear much more natural, lifted, and less boxy. Lastly, the lift lasts longer because the lollipop doesn't rely on the skin to hold the breast up, its actually breast tissue thats being brought together to support the breast and hence improves longevity.

Yes, you would be a good candidate for a lift. It would fix the sagging that you have, as well as the asymmetry. You can use the implants for volume at the top, and remove some of the breast tissue that is sagging at the bottom, which will result in smaller volume but equal, if not better fullness. You can replace your implants at the same time since they are 13 years old and only have a finite lifespan, so you would be "resetting the clock." Talk to your surgeon about the options in person to get a better idea of what's possible, but it should work out well. Best of luck!

The technique I recommend is explantation and lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. Later, fat transfers can be performed if additional volume is desired.

It appears that you would need a significant pocket modification which would include an internal bra, otherwise known as capsulorrhaphy as well as a lift-mastopexy.
