Hello. I am 49 and had breast augmentation in 2018 (IDEAL 370cc/385cc), taking me from a 36B to a 36D. I am unhappy with my nipple location and side profile, but want my nipple closer to the mound, a perkier breast, and more roundness on the bottom. Crescent lift, breast lift, or slightly larger implant? I'm contemplating a 100cc increase to achieve a better profile and bottom roundness. Also wondering if a lower pole placement would be better, they are currently high pole under muscle.
Answer: Breast lift An in person evaluation is best, but from your photos and description of your goals it appears a breast lift is needed. This will remove the excess tissue, giving you a perkier look and will raise the nipple position. You can elect to exchange the implants if you prefer at that time. See a board certified plastic surgeon near you to learn more.

Answer: Implants You need a lift since your breasts are sagging, but I do not recommend going larger. It is possible that these implants are still sitting a little high. measurements are needed to be certain.

Answer: Revision surgery Dear Beppers0327, breast revision surgery is the most difficult plastic surgery procedure performed. Botched breast surgery alters normal tissue planes and laxity, the ability of breasts to heal properly, and leaves scar tissue that dramatically affects the level of difficulty of the corrective surgery. Many patients come in to correct poor outcomes from their initial surgeries. The most common problems are due to implant malposition, bottoming out of the implant, poor scarring, and incorrect implant size. While the type of revision required will depend on the result of the previous surgery and the desired result of the patient, some revisions involve extensive pocket repair that may or may not need specialized external bras.If you are considering breast surgery revision, you should speak with a board certified plastic surgeon and have yourself properly assessed. Only after a thorough examination you will get more information and recommendations. Also, you have to make sure that your surgeon understands your breast augmentation goals. Answer: Explant with Bellesoma Method At this time, the technique I recommend is explantation and lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. Later, fat transfers can be performed if additional volume is desired. Best Wishes, Gary Horndeski, M.D.

Answer: Waterfall Deformity Using a larger implant will not change breast mound shape. Your abundant tissue overlying the implant is ptotic and hanging off the edge of the implant. Only a true breast lift will give you proper nipple position and reveal more lower pole fullness.
