While an implant with a lift would likely give you a more reliable result, it is not the wrong answer to begin with an implant only. In particular, I would utilize a high projecting implant that fits the width of your breast in order to elevate the nipple as much as possible. If you are not happy with the way that this looks, you can always opt for a lift at a later time. In general staging the procedures, with the augmentation and mastopexy (lift) at different times, is also a way to more reliably get you better breast symmetry.

The implants need to be centred behind your nipples, which are located below your breast fold. As a result, the implants would sit rather low on your chest. For this reason, a breast lift is recommended.

I think you can have breast augmentation without a lift. Although it’s more “ideal” to have a lift simultaneously, with an implant that isn’t too large, you can have a very nice and natural result without the lift. At some point, however, your breasts will continue to sag and you will want to make them more youthful by having a lift. If you have the lift now, you can have that more perky look from the beginning, but the trade-off is more scars.
There are three possible outcomes for you without a lift: waterfall deformity with the appearance of high riding implants, waterfall deformity with double bubble, or rock in sock deformity with low riding implants. The reality is your breasts will be optimized in appearance with a lift and the attendant scars associated with it. Going to an expert will minimize scar visibility and most women don’t even see their scars after a while.
Great question. Implants alone are not worth the hazle and here’s why. Implants might give the ilusión of a lift sometimes, but they actually never lift. They only fill out the breasts. In your case implants will fill out the top portion of your breasts only. The bottom portion, which is where most your breast tissue is, including your areola will still sag bellow the implants. Sometimes we can avoid this by using very big implants. But I feel your areola is so low for very big implants to work. Maybe maybe large implants with a circumferential lift is a good middle of the round option. This only leaves a scar around the areola so you actually get a rounder prettier areola.
