Revision rhinoplasty is more difficult than a primary rhinoplasty due to the fact there has been previous alterations to the normal anatomy, and scar tissue present. The bridge line  of the nose can be built up, hopefully with patient  own nasal cartilage harvested from the septum. The hanging columella can also be reduced and the tip de-rotated. The inverted V is best addressed with osteotomies  and a spreader graft placement. The osteotomies will also accomplish straightening the  nasal bones.
