Visible cartilage graft and dent after rhinoplasty, what options do I have to correct these issues? (Photos)

From the photos it appears that the graft is too large to be easily removed through a small office procedure. Fat or filler can fill in the dent and smooth the nose, but will not correct the graft. I prefer fat, because the results are long lasting, while the filler is very temporary.  Any revision needs to wait a full year after your first surgery.  Unfortunately usually the best approach for significant asymmetry is a formal revision where the skin is re elevated, so the cartilage framework can be precisely revised.

It may be possible to remove that cartilage graft with a local procedure, avoiding another big rhinoplasty surgery. Then, once the swelling resolves, if a dent is noticed, not surgical rhinoplasty with either Juvederm or Silicone can be done to fill in defects. It is usually pretty successful.

My first inclination is like yours - avoid another nasal surgery. Depending on your examination, permanent nonsurgical rhinoplastic treatments may be considered for filling the indentation and camouflaging the edge of the graft. Wishing you well moving forward. Dr Joseph
