I'm looking for a super dramatic change in my nose shape. Right now, my nose is wide, bulbous and rounded at the tip, and has a crook in it. I'm looking for a surgeon who will alter it completely - not just attempt to complement my natural features. I've talked to a few plastic surgeons already and they seem to want to go the natural route which is nice and all but not what I want. Are there any surgeons in the Boston area that can help me go from big and bulbous to pretty and petite?

Answer: You should be able to get a large amount of improvement. See the morph I made from your photos. I narrowed the tip of your nose. Your two tip cartilages are strong, and modifying the cartilages can make a big difference in your nose, like in the lady in the short video just above.Megan Fox has thinner skin than you do, and your thicker skin would put a limit on the *amount* of narrowing that you can achieve: your tip can't get as narrow as hers, but what you need is an operation that can get you as much improvement as your personal anatomy will allow. You should understand that the changes I demonstrated in the morph require advanced techniques, techniques that most plastic surgeons cannot handle. Be sure to read the section in the "Web reference" link on how to stay out of trouble while searching for a rhinoplasty surgeon. You should only have surgery if you are able to make yourself very confident in your surgeon's skills. Read that page very carefully. When you see his goals, you'll know whether he has an eye for a beautiful nose, and whether he shares your opinion of what constitutes a beautiful nose. You'll also know whether the changes he proposes are enough to be meaningful to you, and whether he understands your wishes enough to address all of your priorities. But remember, you're not hiring him for his skills with the computer. The doctor must then show you his before and after photos to prove that he can actually accomplish what he draws on the computer. In your case in particular, you need to find a surgeon who understands all of the issues I mentioned, and how they fit together, and who can actually accomplish them in surgery.

From your photographs, it seems like you would be a great candidate for a tip rhinoplasty where your nasal tip cartilage would be modified to add definition to the tip, and make it more refined and petite as you mentioned. I would be happy to talk to you about this further. In general though, when pursuing a rhinoplasty consultation, be sure your surgeon addresses all your aesthetic goals and concerns specifically, and discusses the surgery details including the position and size of the incisions used (with actual photographs and not just digital animations), the risks and benefits of the procedure, recovery time, and post-operative course. Also, be sure to see plenty of before-and-after pictures in women who have a similar nose appearance to yours, and preferably at different time frames (3 weeks out, 3 months out, etc) to more accurately predict your own surgical outcome. And lastly, I think the most important component of the consultation is to establish a good connection with your plastic surgeon. It’s the connection that allows you to clearly define your desired nose outcome goals, the first step of this whole process.
